Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to deliver skilled nursing services in patients' homes across central Illinois. You will coordinate care plans with an interdisciplinary team, educate families, and perform comprehensive nursing assessments.

You will manage wound care, PICC lines, and IV infusions, while documenting thoroughly and ensuring regulatory compliance. Flexible scheduling and autonomous practice are emphasized within a supportive network.

Qualifications

• IL RN license and BLS within 30 days are required.
• College nursing degree or diploma required.
• 1 year nursing experience preferred.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform skilled nursing assessments and wound care including wound vacs and dressings.
• Manage PICC lines and IV infusions; monitor and educate patients and families.
• Lead and collaborate with an interdisciplinary home care team; document promptly.
• Ensure compliance with state and federal regulations and policy guidelines.
