Job summary

3HC Home Health & Hospice Care Inc. in North Carolina is seeking a Registered Nurse (RN) to administer skilled nursing care to patients requiring intermittent professional nursing services.

The role includes weekend coverage with a 40-hour weekend schedule and on-call duties, plus orientation/training Monday–Friday for the first 90 days. The home health RN will adhere to the agency's compliance program and perform comprehensive patient assessments, care planning, and documentation to support

Qualifications

Licensed RN in NC required.
Minimum 1 year of professional nursing experience (acute care preferred).
CPR certification must be current.
Basic computer skills and EMR familiarity required.
Valid driver's license and reliable transportation required.

Responsibilities

Provide skilled nursing care per physician plan of treatment and agency policies.
Document patient status and care provided in medical records and CMS-required forms.
Assess patient needs, adjust care plans, and coordinate with physicians and family.
Educate patients and families on disease process, home care techniques, and self-care.
Collaborate with supervisor and other staff to meet productivity and quality goals.
Participate in discharge planning and care coordination across settings.
