Personal Home Care of North Carolina is hiring Home Health Registered Nurses (RN) for weekend Baylor shifts in Mecklenburg/Union Counties. The role focuses on delivering home health nursing care, patient education, and care coordination under physician supervision.

Qualifications

• Prior home health experience preferred.
• OASIS knowledge required.
• Excellent time management, organizational, and reporting skills.
• Strong written and verbal communication.
• Proficient with technology and health systems.
• Current NC or multi-state RN license.

Responsabilités

• Provide nursing care, assessments, treatments, and patient education per plan of care.
• Observe signs, drug responses, and changes in condition with follow-up as needed.
• Coordinate care and discharge planning with patients, physicians, and teams.
• Document accurately and timely according to regulations.
• Participate in multidisciplinary team meetings weekly.
• Engage in learning and on-going job enhancement activities.
• Participate in On-Call rotation; one weekday on-call required.
