Job summary

Enhabit Home Health & Hospice is seeking a Full-time Weekend Registered Nurse to provide skilled nursing visits in the home, coordinating patient care and collaborating with a multidisciplinary team. The role requires weekend availability, with Saturday and Sunday shifts mandated.

The RN will oversee visit frequency, communicate care details to physicians, perform OASIS data collection, and ensure timely lab reporting while participating in weekly case conferences and monthly meetings.

Qualifications

• Active RN license in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Performs skilled nursing visits and coordinates client care.
• Acts as point of contact for all disciplines involved with providing care to patients.
• Oversees frequency of visits for the episode.
• Consults with the physician and office regarding patient care details.
• Performs accurate OASIS collection and maintains current medication profile.
• Ensures lab values are reported to the physician timely.
• Attends weekly case conference and monthly case manager meetings.
