Elite Home Health is seeking an experienced Registered Nurse (RN) to provide and coordinate skilled nursing care in patients' homes across Sarasota, Florida. Qualifications

• RN licensure in Florida in good standing.
• ASN or BSN required.
• Minimum 1 year nursing experience.
• Home health, hospice, or acute care preferred.
• Current CPR certification.

Responsibilities

• Perform comprehensive and ongoing patient assessments.
• Develop, implement, and update individualized patient care plans.
• Coordinate care with physicians, therapists, and interdisciplinary team members.
• Complete accurate, timely clinical documentation including OASIS assessments.
