Job summary

Empath Health is seeking a compassionate PRN Home Health RN to join our north Sarasota County team. This role delivers individualized in-home nursing care, performing comprehensive assessments and developing plans of care alongside a supportive interdisciplinary team.

Ideal candidates hold a Florida RN license, with BSN/MSN preferred, and at least one year of direct patient care experience, particularly in home health. Weekend/holiday on-call rotation participation is required.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
