Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care in patient homes across our service area. You will perform assessments, communicate with families, and collaborate with an interdisciplinary team to promote independence and wellness.

The role offers autonomy with 24/7 clinical backup, flexible scheduling and mileage reimbursement. Requires an IL RN license, nursing diploma, and relevant home health experience.

Qualifications

• Active IL RN license and BLS certification.
• 1+ year nursing experience in home health or skilled care.
• Ability to work independently within an interdisciplinary team.

Responsibilities

• Coordinate and provide direct and indirect patient care per plan.
• Perform complex wound care including wound vac and dressings.
• Monitor central lines such as PICC/Hickman; change dressings per protocol.
• Complete OASIS/485 changes within 24 hours of admission.
• Communicate with MD and care team to adjust plans and updates.
