Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care across multiple counties. You will educate patients and families, perform comprehensive assessments, and supervise home health aides as part of an interdisciplinary team.

The role includes wound care, PICC line management, and coordination with physicians and care coordinators to ensure timely, quality home health services. Travel is involved within the service area.

Qualifications

• Illinois Registered Nurse (RN) license required.
• BLS certification within 30 days of hire.
• Valid driver's license and proof of auto insurance.
• College diploma in nursing and at least 1 year of nursing experience.

Responsibilities

• Coordinate and provide direct patient care using the nursing process.
• Perform wound care, including wound vac and ostomy care.
• Care for PICC lines, ports, drains, and catheters per protocol.
• Complete OASIS/485 assessments and timely documentation.
