SA Nurses is expanding its home health team and seeks dedicated home health nurses to provide in-home care, often after hospital discharge. You will assess patients, perform vital signs, wound care and IV therapy, and coordinate with therapists and physicians to implement the patient care plan.

We value compassionate, patient-centered care, strong communication, and efficient time management to support patients in their homes. Medical insurance and PTO are provided for full-time employees.

Responsabilités

• Assess patients' condition during each visit and document observations.
• Perform evaluations including vital signs, medication review, Oasis assessment, wound care and IV therapy.
• Dress or redress wounds and monitor healing progress.
• Collect data and prioritize problems and needs.
• Supervise CNAs and LVNs on assigned caseload.
• Document to company and compliance standards.
• Educate patients and families on home health care strategies.

Home Health Nurse Responsibilities

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review, Oasis assessment, wound care and IV therapy.
• Dress or redress open wounds and assess progress of healing
• Collect data and prioritize problems and needs
• Supervise CNA and LVN under assigned to your caseload
• Document to company and compliance standards
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
