Job summary

SA Nurses is hiring Home Health Nurses to provide at-home nursing care following hospital discharge or post-acute care. You will assess patients, administer care, and educate families in a compassionate, patient-centered approach.

Responsibilities include vital signs, wound care, medication review, and collaboration with therapists and physicians to optimize outcomes.

Qualifications

• Clinical knowledge and compassionate care are essential.
• Strong time management and interpersonal communication required.
• Ability to educate patients and families on home health care best practices.

Responsibilities

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review, Oasis assessment, wound care and IV therapy.
• Dress or redress open wounds and assess progress of healing
• Collect data and prioritize problems and needs
• Supervise CNA and LVN under assigned to your caseload
• Document to company and compliance standards
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
