Graham Healthcare Group is seeking a Weekend Physical Therapist for our Home Health division. This role offers the chance to provide skilled care to patients in their homes in Troy and surrounding areas.

Qualifications

• Active Physical Therapist, PT license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver’s license required for home visits.

Responsabilités

• Deliver skilled, patient-centered care in home health.
• Perform comprehensive patient evaluations and develop therapy plans.
• Educate patients and caregivers on exercises and safety.
• Think critically and ensure compliant EMR documentation.
• Travel to patient homes within assigned territory.
