Qualifications

• Licensed Physical Therapist issued by the state where you will work.
• Minimum of one year healthcare experience.
• Valid driver’s license and ability to spend ~20% of day driving to/from patient locations.
• Commitment to meeting critical deadlines for charting.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Deliver and manage patient care plans in home health settings.
• Collaborate with AHN Healthcare@Home team to coordinate care.
• Document care and visits using EMR software.
