Job summary
Graham Healthcare Group is seeking a Home Health Physical Therapist to provide skilled care in patients' homes around Independence, MO. This role offers autonomy within an interdisciplinary team and opportunities for meaningful patient relationships.
The ideal candidate holds an active PT license in the state, with at least 1 year of PT experience and a valid driver’s license for travel to patient locations. A $10,000 first-year success bonus is available, with additional long-term rewards.
Qualifications
- Active PT license in the state you work.
- At least 1 year of PT experience delivering patient care.
- Valid driver's license with ability to drive ~20% of the day.
Responsibilities
- Deliver skilled, patient-centered care in home health with an interdisciplinary team.
- Perform evaluations, mobility assessments, and develop therapy plans to improve strength, balance, and independence.
- Educate patients and caregivers on exercises, injury prevention, and safe mobility at home.
- Think critically, solve problems in real time, ensure compliant EMR documentation.
- Work independently, but collaborate with team when needed.
- Travel to patient homes within assigned territory.
