Job summary

Graham Healthcare Group in Van Voorhis, PA is seeking a Home Health Physical Therapist to join our AHN Healthcare@Home partner team. Build patient care plans, deliver therapy in patients’ homes, and help manage ongoing therapy programs across Monongahela and surrounding areas.

This staff position offers 40 hours/week, a $10,000 first-year bonus, competitive pay per visit, 401(k) with matching, and generous holiday time.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state you will work in.
• Minimum of one year healthcare experience.
• Must have a valid driver’s license and ability to drive ~20% of the day.
• Ability to meet deadlines for charting and manage your schedule.

Responsibilities

• Deliver home-based physical therapy plans and adjust treatments as needed.
• Collaborate with AHN Healthcare@Home team to manage patient care plans.
• Assist in managing and delivering patient care in patients’ homes across Monongahela, PA and surrounding areas.
