Job summary

HealthPro Heritage in Groton, CT is seeking a dedicated Physical Therapist to join our home health team serving Groton and surrounding areas. You will evaluate and treat adults, educate patients and families, document progress, and collaborate with a multidisciplinary team.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Valid state licensure as a Physical Therapist or license eligible.
• Strong clinical assessment, treatment planning and patient communication.
• Ability to lift and assist patients and perform therapy tasks.

Responsibilities

• Evaluate patients' physical conditions including mobility, strength and flexibility.
• Provide therapeutic exercises, manual therapy and patient education to aid recovery.
• Maintain accurate documentation of evaluations, progress and plans.
• Collaborate with healthcare providers to ensure coordinated patient care.
