Job summary

HealthPro Heritage at Home is seeking a licensed Physical Therapist for a PRN Home Health position in Newington, CT and surrounding areas. You will evaluate and treat adults in patients' homes, create individualized plans, and coordinate care with a multidisciplinary team.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain; educate on home routines.
• Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with standards.
• Collaborate with healthcare providers to ensure effective patient care and participate in multidisciplinary meetings.
