Job summary

OptumCareCare caregivers, part of the LHC Group family, are seeking a weekend Baylor RN for Home Health in Massachusetts. This position covers 32 hours per week and involves managing a plan of care, supervising home health staff, and performing comprehensive in-home nursing care under physician direction.

Responsibilities include OASIS assessments, medication reconciliation, and coordinating with physicians and families to ensure continuity of care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations.
• Provides supervisory visits as required.
• Completes OASIS assessments including medication reconciliation and documents patient visits per policy.
• Contributes to quality care and patient outcomes through timely communication and coordination.
