Job summary

Harmony Home Health Service seeks a registered nurse to plan, organize and direct home care services with emphasis on community health education for individuals and families in their homes. Qualifications

• Graduate of an accredited school of nursing (Diploma or transcripts provided).
• Current state licensure, CPR certification, valid driver's license.
• One year nursing experience recommended; home health care experience preferred.
• Criminal background check must be completed before hire.
• Complete and pass skill evaluation test.

Responsibilities

• Completes initial patient and family assessment to determine home care needs and performs OASIS assessments as appropriate.
• Regularly re-evaluates patient nursing needs and revises the plan of care as status changes.
• Initiates and revises care plans with goals based on nursing diagnosis, involving patient and family.
• Coordinates services, makes referrals, and administers prescribed medications/treatments.
• Counsels patient and family in meeting nursing and related needs.
• Communicates with physician and staff, updates progress notes at intervals (e.g., every 60 days).
• Acts as Case Manager when assigned and coordinates care for a caseload.
• Supervises home health aides and collaborates with interdisciplinary team.
