Qualifikationen

• Graduate of an accredited school of nursing with diploma or transcripts provided to agency.
• Current licensure in state, CPR certification and valid driver’s license.
• Minimum of one year nursing experience; home health care experience preferred.
• Health certificate and TB test must be current; criminal background check required.
• Ability to supervise home health aides; strong observation and communication skills.

Aufgaben

• Complete initial assessment of patient and family to determine home care needs including OASIS assessments at appropriate time points.
• Regularly re-evaluates patient nursing needs.
• Initiate the plan of care and revise as patient status changes.
• Use health data to determine nursing diagnoses and provide required services.
• Develop and implement care plans with patient/family involvement.
• Coordinate services and make referrals as needed.
• Administer medications and treatments as prescribed by physician.
• Counsel patient and family on nursing needs and health plans.
• Prepare progress notes and update physician at least every 60 days.
• Assign home health aides and collaborate with interdisciplinary team.
