Job summary

Graham Healthcare Group in Washington, DC is seeking an Intake Coordinator for weekend coverage to join our dynamic team. You will interface with referral sources and ensure referrals are received with a clear understanding of the clinical and service needs of our patients.

The role requires Home Health experience and customer service skills, with duties including processing referrals, basic diagnosis coding, coordinating care using the Welcome Call Script, and supporting the Clinical Intake

Qualifications

• Associate’s Degree preferred.
• At least one year experience in Home Health preferred.
• At least one year of experience in a customer service capacity.

Responsibilities

• Process agency home health referrals accurately and timely.
• Complete basic diagnosis coding per referral information.
• Coordinate care for homecare patients using the Welcome Call Script.
• Determine referrals from the Clinical Intake team.
