Job summary

Tanner Health is seeking an intake coordinator for weekend coverage (Friday–Monday) to handle referrals for Home Health. You will coordinate initial in-home assessments and act as the primary communication link among hospitals, physicians, and referral sources.

The role requires strong organizational and communication skills, with experience in healthcare intake and familiarity with Maestro and Meditech a plus. High School Diploma and vocational training or GED acceptable.

Qualifications

• High School Diploma plus 1 year of vocational school or GED with related experience.
• One year of healthcare intake or scheduling experience preferred.
• Proficient computer skills; knowledge of Maestro and Meditech is a plus.
• Strong communication and organizational abilities required.

Responsibilities

• Accept telephone referrals and coordinate initial in-home assessments for patients.

This position is for weekend coverage (FRIDAY-MONDAY)

Responsible for accepting telephone referrals and coordinating initial in-home assessments for patients that are referred to Home Health. This individual will be the first point of contact for the agencies and will be responsible for communicating and coordinating patient care with hospitals, physician offices, and other referral sources.

Education

High School Diploma plus 1 year of vocational school

Experience

One year of related experience. Requires a working knowledge of standard practices and procedures.

Qualifications

• Good organizational skills are required.
• Good verbal and written communication skills are required.
• Must have proficient computer skills. Experience with Maestro and Meditech clinical application systems preferred. Prefer a minimum of two years of experience working in healthcare with knowledge of in-take and patient scheduling and insurance verification.
• Will consider GED with 1+ years of vocational school and 1+ years of medical office experience.
