Mass General Brigham Home Care is seeking a Physical Therapist to join the Weekend team in a hybrid, part-time role supporting communities south of Boston. You will evaluate patients, develop personalized treatment plans, and deliver one-on-one PT services in patients' homes on Saturdays and Sundays.

Expect focus on mobility, safety, and independence, with collaborative care among nurses, physicians, and social workers.

Responsibilities

• Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes.
• Daily documentation is accurate and timely.
• Collaborate with nurses, therapists, physicians, and social workers to support recovery.
• Provide physical therapy services to patients in their residence; promote safety and independence.
