Mass General Brigham Home Care, Inc. is seeking a Physical Therapist to deliver 1:1 in-home care across Lynn, Saugus, and Nahant, with a part-time 32-hour schedule.

You will evaluate patients, craft personalized treatment plans, and collaborate with an interdisciplinary team to support recovery at home. Requirements include at least 1 year of acute care/rehab/homecare experience, Massachusetts licensure, strong communication and documentation skills, and willingness to travel within the MBH Home

Qualifications

One year of experience in acute care, rehab or homecare as a Physical Therapist.
Registered with the Board of Registration in Massachusetts.
Excellent verbal, written and electronic communications skills.
Computer proficiency and ability to learn new software.
Ability to prioritize work and adapt to changing situations.
Travel required within Mass General Brigham Healthcare at Home's geographic area.

Responsibilities

Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes.
Daily documentation is accurate and timely.
Application of standard treatment procedures.
Teaching is provided to patients and caregivers.
