Job summary

Mass General Brigham Home Care, Inc. seeks a licensed Physical Therapist to join our Chelsea, MA home-care team in a hybrid, full-time capacity. You will provide 1:1 physical therapy in patients’ homes, focusing on mobility, safety, and independence.

You will evaluate patients, develop individualized treatment plans, and collaborate with nurses, physicians, and social workers. MA licensure and at least one year of home or acute rehab experience are required.

Qualifications

• One year of experience in acute care, rehab or homecare as a Physical Therapist.
• Massachusetts Physical Therapist license required; registration with the Board of Registration in Massachusetts is required.
• Excellent verbal, written and electronic communications skills; ability to learn new software.

Responsibilities

• Deliver 1:1 physical therapy care in patients’ homes as a Case Manager.
• Evaluate patients and develop personalized treatment plans.
• Focus on mobility, strength, fall prevention, and functional independence.
• Assess home environments to promote safety and independence.
• Educate patients and caregivers on recovery strategies and long-term mobility.
• Collaborate with nurses, therapists, physicians, and social workers to support recovery.
