Job summary

Qualifications

• Master of Social Work from a CSWE-accredited program.
• Virginia licensure as Licensed Clinical Social Worker preferred.
• One year of clinical social work experience required (CSW Level 2).
• Experience in an acute care setting preferred.

Responsibilities

• Complete psychosocial assessments for patients and families.
• Support care planning and coordinate discharge planning services.
• Provide psychosocial intervention and education to patients, families, and staff.
• Educate inter-disciplinary team and community resources.
• Develop intervention plans and monitor progress.
• Participate in quality improvement activities and meetings.
