Qualifications

• CSW Level 2: one year of clinical social work experience.
• Master of Social Work from a CSWE-accredited program.
• Clinical social work experience in an acute care setting preferred.

Responsibilities

• Complete psychosocial assessments.
• Support care planning.
• Provide psychosocial intervention.
• Educate patients, their families, staff and interdisciplinary team members.
• Coordinate discharge planning services.
• Facilitate treatment compliance.
• Work with the community to provide needed services.
