Qualifications

• Must have strong communication abilities to work with the team and the individuals served.
• Driver's license required for transportation duties.
• High school diploma or equivalent is expected.
• Willingness to learn and adapt in caregiving environments.

Responsibilities

• Collaborate with the HCBS Supervisor and Programmer to develop individualized goals and provide direct support that focuses on teaching skills and encouraging participation in activities.
• Use approved behavioral strategies to support individuals while respecting rights as outlined by the plan.
• Complete accurate, timely documentation in Setworks, ensuring all service details and responses are recorded.
• Provide hands-on support in home and community settings, including daily living assistance and transportation.
• Administer and document medications per policy, maintain reliable attendance including weekends/holidays.
