University of Texas MD Anderson Cancer Center in Houston, TX seeks a Case Manager Navigator to integrate utilization management, care facilitation and discharge planning for complex patient transitions. You will coordinate with physicians, nurses, social workers, payors and community agencies to optimize outcomes and patient experience.

The ideal candidate holds a BSN, 5 years of professional nursing (4 years if oncology or case management experience), an active Texas RN license and BLS; CCM/ACM

Formación

• BSN is required; 5 years nursing experience (4 years if oncology nursing/case management).
• Texas RN license and BLS required; CCM/ACM certifications preferred.

Responsabilidades

• Assess patient needs based on models for high-risk or complex care.
• Coordinate, facilitate, and monitor care across the continuum for optimal outcomes.
• Oversee discharge planning and safe transitions to community settings.
• Intervene, advocate, and adjust care plans for patients.
• Liaise with multidisciplinary teams, physicians, payors, and community agencies.
• Identify strategies to improve processes and reduce costs; support patient care and business outcomes.
• Other duties as assigned.
