Qualifications

• BSN required for consideration.
• Texas RN license and current BLS certification required.
• CCM or ACM certification preferred.
• Weekend availability for four 10-hour shifts.

Responsibilities

• Assess patient needs and coordinate high-risk or complex care.
• Coordinate and monitor care across the continuum to optimize outcomes.
• Oversee discharge planning and safe transitions to the community.
• Advocate and adjust care plans as needed for patients.
• Serve as liaison among multidisciplinary teams and external partners.
