Job summary

Responsibilities

• Assess patients for case management needs and discharge planning.
• Develop and evaluate individualized discharge plans and care transitions.
• Coordinate services such as home health, DME, transportation, SNF, and rehab referrals.
• Maintain timely documentation of assessments, interventions, and discharge arrangements.
• Collaborate with the health care team and Utilization Management.
