Advocate Health's Aurora Psychiatric Hospital is seeking a part-time Case Management professional to coordinate discharge planning and support families and care teams. The role requires nursing or social work licensure in Wisconsin, and experience in behavioral health and utilization review.

Qualifications

• Requires RN license or Social Worker license or Professional Counselor license (Wisconsin)
• Bachelor's in Nursing or Master's in Social Work/ Psychology required
• Experience in behavioral health and utilization review preferred
• Ability to lead and coordinate discharge planning
• Strong written and oral communication skills

Responsibilities

• Coordinate discharge planning and utilize specialty services as needed
• Monitor discharge plans and ensure timely completion
• Communicate with families, physicians, therapists and payors
• Explain estimated benefits and assist with financial options
• Initiate denial/appeal process when required
• Collaborate with treatment teams to complete discharge plans on time
