Job summary

Sevita in Evansville, IN seeks a Direct Support Professional to provide direct support within a single program or unit, typically assisting 4–6 individuals in residential settings or up to 10 individuals in periodic programs.

You will assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living, while maintaining confidentiality and following service plans.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Direct Support: provides direct support per program plans; assists with socialization, personal care, housekeeping, transportation, community orientation, shopping, citizenship, and activities of daily living.
• Records: documents progress and activity; maintains confidentiality.
• Management of Individuals’ Assets: makes cost‑effective purchases within the budget and according to preferences.
• Relationships: maintains professional relationships with individuals, families, guardians and case managers; follows Sevita Customer Service Standards.
• Reporting: reports abuse/neglect; reports medical and other incidents per policy.
• Rights: maintains confidentiality and respects rights; supports universal precautions.
