Qualifications

• Active and unrestricted Florida RN license.
• Medicare-certified home health and OASIS experience preferred.
• Current CPR certification.
• Strong assessment, communication, and time-management skills.
• Ability to work independently and manage a weekend caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing visits to home health patients Friday through Monday.
• Complete patient assessments and develop individualized plans of care.
• Perform admissions, Start of Care, resumptions of care, recertifications, routine visits, and discharges.
• Provide wound care, medication management, education, and postoperative support.
• Monitor patients’ conditions and communicate changes to physicians and team.
• Educate patients and caregivers on medications, treatments, safety, and disease management.
• Coordinate care with physicians, therapists, caregivers and other team members.
• Complete accurate and timely clinical documentation.
• Assist with urgent weekend patient needs within the territory.
