Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate Weekend Baylor RN Registered Nurse to deliver home health care in Lee County (Fort Myers, Bonita Springs, Estero and nearby areas). You will perform comprehensive patient assessments, develop plans of care, and educate families while collaborating with the care team to provide Full Life Care.

The role requires Florida RN licensure, a graduate of an approved nursing program, and at least one year of direct patient care; BSN/MSN

Qualifications

• Graduate of an approved nursing program and current Florida RN license.
• BSN or MSN preferred.
• Minimum of one year direct patient care experience; home health RN experience preferred.

Responsibilities

• Perform comprehensive patient assessments including required OASIS assessments.
• Develop and revise individualized plans of care.
• Deliver skilled nursing services per plan of care.
• Counsel and educate patients and families on disease processes and self-care.
• Collaborate with the interdisciplinary team to ensure holistic care.

Empath Home Health , a member of Empath Health is seeking a compassionate Weekend Baylor RN Registered Nurse - Home Health to work with our Home Health team to deliver individualized care to patients in their homes throughout Lee county (Fort Myers, Bonita Springs, Estero, and surrounding areas).

Locations: Home health visits in Lee county (including Fort Myers, Bonita Springs, Estero , and surrounding areas).

Schedule: This is a Weekend Baylor role requiring full weekends . You decide which schedule below fits your life best!

• 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
• 3 days (Friday-Sunday OR Saturday-Monday)

What you’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
• BSN or MSN preferred.
• Minimum one year experience providing direct patient care; previous experience as a home health RN preferred.
• Home Health RN experience required
