Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN to deliver individualized care to patients in their homes across Pinellas County including Clearwater, St. Petersburg, Palm Harbor and surrounding areas.

This role requires completing comprehensive patient assessments and developing plans of care. Schedule is Monday through Friday, 40 hours, with weekend/holiday on-call rotations.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
