Empath Home Health, a member of Empath Health, is seeking a compassionate Weekend Baylor RN to provide in-home nursing care across North Hillsborough and East Pasco counties including North Tampa, Dade City, and Land O’Lakes.

The role is part-time, requiring full weekends (Saturday and Sunday) for 20 hours weekly. The RN will perform OASIS assessments, develop care plans, and educate patients while coordinating with the health team.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience.
• Home Health RN experience required.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
