Job summary

The Brien Center in Pittsfield, MA is seeking a Part-time Family Supportive Living Case Worker to provide direct case management services to families in the Residential Program, promoting independence, skill-building, and attainment of family goals.

You will collaborate with families, document daily interactions, and participate in supervision, while building connections with medical and community supports.

Qualifications

• High School Diploma or equivalency required; a Bachelor’s degree or certification in human services preferred.
• Basic knowledge of community resources.
• Ability to be professional and manage confidential information.
• Ability to advocate program services to community members.
• Ability to communicate effectively and be organized.

Responsibilities

• Adheres to all applicable state and federal regulations; ensures compliance with the organization’s policies and procedures, code of conduct, and all applicable regulatory requirements.
• Collaborates with families to create specific short- and long-term goals with timelines. Monitors progress towards goal attainment and modifies goals as necessary to enhance an individualized treatment plan to best serve the family.
• Collaborates and participates in meetings and service coordination with outside agencies and community supports.
• Develops strong working relationship with medical practitioners, outpatient clinicians, and other community supports.
• Encourages and supports clients in developing a natural and long-term support network.
• Maintains daily documentation of resident interactions and case management appointments. Follows and monitors medication observation policy for resident medication regimens.
• Develops and facilitates a variety of groups to meet needs of the residents.
• Participates in weekly supervision with Program Manager.
