Qualifications

• Experience in healthcare admissions, case management, or marketing preferred.
• Knowledge of skilled nursing or long-term care a plus.

• Ability to work independently and make timely decisions.
• Reliable transportation for local travel within Boston (if applicable).

Responsibilities

• Respond promptly to weekend referrals from hospitals and other sources.
• Review clinical documentation to determine admission eligibility.
• Coordinate with clinical and administrative teams to facilitate smooth admissions.
• Communicate effectively with families regarding admission process and expectations.
• Maintain accurate documentation and tracking of referrals.
• Represent the facility professionally with referral partners.
