Job summary

Horizon Home Health Care is seeking a compassionate RN to deliver high-quality home health services in Clark County, Ohio and surrounding areas. The role covers SOC through discharge, with autonomy in managing patient care and care plans.

You will collaborate with an interdisciplinary team and ensure OASIS documentation complies with Medicare guidelines. Excellent communication and time management are essential for success in this flexible, patient-centered environment.

Qualifications

• Current Ohio RN license.
• Experience in home health and OASIS assessments preferred, but not required.
• Knowledge of Medicare home health regulations and documentation requirements preferred, but not required.
• CPR/BLS certification.
• Valid driver’s license with reliable transportation and proof of auto insurance.
• Strong assessment, communication, organizational and time management skills.

Responsibilities

• Performs Start of Care (SOC), Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team.
• Monitors patient progress and adjusts care plans as needed.
• Completes accurate and timely OASIS documentation and clinical records per Medicare and agency guidelines.
• Participates in quality improvements initiatives and maintains regulatory compliance.
