Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to support patients across the Greater Sacramento Area. You will perform SOC assessments, coordinate with the clinical team, and visit patients in their homes on Saturdays and Sundays, with flexible weekday options.

The role requires an unrestricted CA RN license, BLS/CPR, and a valid CA driver's license. Experience with IV therapies and OASIS is highly valued.

Qualifications

• Must hold current and unrestricted CA RN license.
• BLS/CPR Certification required.
• Valid CA Driver's License.
• Experience with Start of Care (SOC) - OASIS highly desired.
• Strong clinical knowledge in home health care.
• Experience with high acuity patients is a plus.
• Experience with IV medication infusions highly preferred.

Responsibilities

• Utilize your clinical skills to complete initial patient assessments and determine home care needs.
• Conduct SOC admissions and OASIS as appropriate; perform follow-ups and evaluation visits.
• Make home visits within the designated territory.
• Reconcile medications, perform wound care, and manage treatments as needed.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver high-quality care.
• Administer IV antibiotics/diuretics safely and per protocol.
• Document observations, interventions, and patient care activities.

Part time Registered Nurse Case Manager

Anticipated Territory : Greater Sacramento Area

Schedule: Every Saturday and Sunday full days. Would consider Saturday/Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

To be a success:

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
• Previous experience with IV Medication infusions highly preferred
