Qualifications

• High school diploma or equivalent required.
• One year activities experience preferred.
• Two years experience in a long-term care, hospital, or other related healthcare setting preferred.

Responsibilities

• Care Management and Service Delivery - 70%: Provide programming that meets residents' interests and well-being; assist with hobbies and crafts; visit bed-bound residents; routine visits; assist with transporting residents; accompany residents to off-site activities; interact with residents to ensure quality care; maintain relationships with resident families; communicate activity information; provide reading materials as needed.
• Operations Management - 20%: Coordinate parties, movies, games/activities; develop evening activity schedule; complete monthly activity logs; communicate needs for supplies; attend meetings and trainings.
• Quality, Compliance, and Risk Management - 10%: Develop and oversee plans of care; ensure charts and documentation are complete and compliant; maintain confidentiality.
