Responsibilities

• Care management and service delivery to provide programming that supports residents’ physical, mental, and psychosocial well‑being.
• Coordinate scheduled parties, movies, games, and activities suitable for residents across levels of care.
• Develop and oversee written plans of care and ensure progress notes are documented.
• Maintain confidentiality of resident information and uphold resident rights.
