The role requires a high school diploma, an active MD driver's license, and the ability to complete documentation accurately. You will assist with medication monitoring, ADLs, and coordination with treatment teams, with travel locally as needed.

Qualifications

• High school diploma - no experience required
• Valid/Active MD Driver's License (obtained for more than 3 years)

Responsibilities

• Provide comprehensive services through on-site and off-site programs, including case management, psycho education, crisis intervention, advocacy, and community liaison.
• Timely and accurate completion of all documentation such as shift logs, incident reports, and service notes.
• Medication monitoring and education to support self-dosing of medication.
• Teaching community skills including ADLs as needed in both individual and group settings.
• A Rehab Counselor will assist in the transportation of clients between residences, office, and community locations as needed.
• Assist the Rehabilitation Counselors and other staff as needed.
• Maintaining their level of skill by completing the required 20 hours per year of training and abide by safety rules and regulations.
