Job summary

Affinity Hospice in Stockbridge, GA is seeking a dedicated Registered Nurse to join our on-call weekday team. You will plan and coordinate in-home hospice care, guiding patients and families with compassion and clinical expertise.

The role requires an active Georgia RN license, CPR certification, and at least two years of RN experience in clinical, home health, or hospice settings. We offer PTO, a gas card program, 401K, and tuition reimbursement, among other benefits.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Georgia.
• Minimum two years RN experience in clinical, home health or hospice.
• Current CPR certification.

Responsibilities

• Case manages and coordinates in-home hospice care for assigned patients.
• Collaborates with interdisciplinary team to implement patient plans of care.
• Documents care and ensures accuracy and continuity of services.
• Provide on-call nursing coverage and patient/family support as needed.
