Job summary

Southwest General Home Health is seeking a dedicated Home Health Aide to support patients in their homes under RN supervision. The role emphasizes compassionate care, safety, and accurate documentation, with regular visits and a Monday–Friday schedule.

You will perform personal care, assist with daily activities, and follow the plan of care to ensure patient comfort and safety. Requires STNA/CNA training, a year of experience, and a valid driver's license.

Qualifications

• High school diploma or equivalent required.
• STNA or CNA training (75 hours) required.
• One year of verifiable nurse aide experience under RN supervision.
• Score 80% or higher on skills competency test.
• Valid driver’s license and auto insurance listing you as a driver.
• Ability to safely perform hands-on care, including lifting and transferring.
• Complete required training, including 12 hours of annual in-services.

Responsibilities

• Assist with bathing, grooming, toileting, and other personal care needs.
• Help with meals, laundry, and light housekeeping.
• Support safe transfers, walking, and prescribed exercises.
• Observe and report changes in patients’ conditions.
• Complete visit documentation within 24 hours.
• Travel to scheduled patient visits and follow each patient’s plan of care within your scope of practice.
