Job summary

Southwest General Home Health is seeking a compassionate Home Health Aide to assist patients at home under RN supervision. You will help with personal care, meals, and daily activities while ensuring safety and comfort.

Requirements include STNA/CNA credential, at least one year of nurse aide experience, a valid driver’s license, and the ability to perform lifts and transfers. Schedule is Monday–Friday with training provided.

Qualifications

• High school diploma or equivalent.
• STNA/CNA credential or completion of a 75-hour nurse aide training program.
• One year of verifiable nurse aide experience under RN supervision.
• Valid driver's license and auto insurance listing you as a driver.
• Ability to lift, transfer, and bathe patients.

Responsibilities

• Assist with bathing, grooming, toileting, and other personal care needs.
• Help with meals, laundry, and light housekeeping as needed.
• Support safe transfers, walking, and prescribed exercises.
• Observe and report changes in patients' conditions.
• Complete visit documentation within 24 hours.
• Travel to scheduled patient visits and follow each patient’s plan of care within your scope.

What You’ll Do

• Complete visit documentation within 24 hours.
• Travel to scheduled patient visits and follow each patient's plan of care within your scope of practice.

What You’ll Bring

• High school diploma or equivalent.
• State Tested Nurse Aide (STNA) credential or completion of a 75-hour nurse aide training program (CNA).
• One year of verifiable nurse aide experience under an RN's direct supervision.
• A score of 80% or higher on the skills competency test.
• Valid driver's license and auto insurance listing you as a driver.
• Ability to safely perform hands-on care, including lifting, transferring, and bathing.
• A caring attitude, strong communication skills, and dependable attendance.

Must complete required training, including 12 hours of annual in-services.
