Qualifications

Responsibilities

• Acting as primary Waiver contact for the family and other professionals via the Plan of Care.
• Monitoring the tasks outlined on the POC with the treatment team and coordinating schedules.
• Coordinating and scheduling respite care.
• Acting as Treatment Plan Coordinator for assigned clients.
• Providing case management, resource acquisition, crisis intervention and outreach activities.
• Delivering services in various locations including home, school and community settings.
• Providing transportation to activities or appointments as indicated.
• Responding to crises while on call in rotation with staff.
• Regular and predictable attendance is essential.

Pay is commensurate with experience within a range of $20.00 to $24.00 per hour. Full time, non-exempt, benefits eligible. Responsibilities

The primary responsibility of SED WaiverFacilitator will be to organize and manage the components of the Waiver Programfor children that meet the criteria for the targeted population. Specific duties include but not limited to :

• Acting as primary Waiver contact forthe family and other professionals working with the client via the client'sPlan of Care/POC (treatment plan);
• Monitoring the tasks outlined on thePOC with other treatment team members, medication services staff, school andother community providers by communicating with such staff and communityproviders on a regular basis and coordinating schedules and assignments;
• Coordinating and scheduling respitecare in accordance with client's needs;
• Acting as Treatment Plan Coordinatorfor assigned clients;
• Providingcase management services, resource acquisition, crisis intervention, outreachactivities and coordinate these services with other community health and socialservice agencies;
• Deliveringservices in a variety of locations, including but not limited to the client'shome, the school, the Child & Family Services program site, or anothercommunity setting, on an as-needed basis;
• Providingtransportation to and from various activities or appointments as indicated bythe treatment plan or as instructed by the QMHP;
• Responding to crises while on call inrotation with other similarly trained staff;
• Regular and predictable attendance is anessential function of the job; and
• Additional responsibilities that may beassigned as circumstances warrant.

Qualifications

The SED Waiver Facilitator position requires but not limited to:

• a bachelor's degree in psychology,social work, psychiatric nursing or another related human services field or beequivalently qualified by work experience or a combination of work experiencein the human services field and education with one year of experiencesubstituting for one year of education.
