Qualifications

• Bachelor's degree in psychology, social work, psychiatric nursing or related human services field, or equivalent through work experience.
• Reliable transportation and a valid, unrestricted driver's license.
• Prior computer experience and basic computer fluency.
• Ability to work independently and in cooperation with others.
• Ability to exercise sound independent judgment.
• Flexibility in assigned working hours.
• Commitment to interdisciplinary and community mental health center approaches.
• Energetic and positive approach to rapidly evolving changes and challenges.

Responsibilities

• Acting as primary Waiver contact for the family and other professionals via the Plan of Care.
• Monitoring the tasks outlined on the POC with the treatment team and coordinating schedules.
• Coordinating and scheduling respite care.
• Acting as Treatment Plan Coordinator for assigned clients.
• Providing case management, resource acquisition, crisis intervention and outreach activities.
• Delivering services in various locations including home, school and community settings.
• Providing transportation to activities or appointments as indicated.
• Responding to crises while on call in rotation with staff.
• Regular and predictable attendance is essential.
