Qualifications

• Bachelor’s degree in social work or related field and active license or equivalent.
• One year experience with developmental disabilities or relevant training.
• Training offered if qualifications do not meet exact requirement.

Responsibilities

• Foster trusting relationships with individuals served with empathy and communication.
• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams for seamless care delivery.
• Connect individuals with community resources to boost independence.
• Maintain up-to-date documentation and ensure compliance with guidelines.
• Complete daily case noting documentation of tasks.
• Collaborate with team to address evolving needs and adjust plans.
• Provide guidance to individuals and families for informed care decisions.
• Travel to meet clients for Annual and Semi-Annual meetings as needed.
