Qualifications

• Bachelor's degree in social work with an active Minnesota social work license or a related field degree.
• One year of experience in the education or treatment of people with developmental disabilities or a course focusing on developmental disabilities (training provided if not).
• Excellent written and verbal communication, strong documentation and travel readiness.

Responsibilities

• Develop, review, and monitor individualized care plans aligned with each client's needs and goals.
• Coordinate services with interdisciplinary teams to ensure seamless care.
• Connect clients with community resources to increase independence and well-being.
• Maintain compliance with lead agency guidelines and accurate documentation.
• Complete daily case noting of tasks performed.
• Travel to meet clients for Annual and Semi-Annual meetings as needed.
